our doctors are on the staff of two
hospitals in Montgomery. We will see
your newborn baby at Jackson Hospital and
Baptist Medical Center East.

MEDICAL ADVICE (MOMS & DADS LINE NURSE)
You may talk directly to our nurse on the
"MOMS & DADS LINE" at 334-272-6667. She is
available to assist with any medical
questions you may have regarding your child,
or help
decide if your child needs to see our
pediatrician. If you know your child needs
an appointment,
please call our regular appointment line
directly. Prescriptions, refills, blue slips
or school forms
should be requested from the MOMS & DADS
line nurse. The advice we can give on the
phone is
limited to our active patients only.
HEALTH FORMS
Please give our MOMS & DADS Line Nurse
twenty-four hours notice for any forms to be
completed such as immunization records, Blue
Immunization Certificates (Blue Slips), day
care
and sports physical forms. Sports physicals
for rapidly changing adolescents are
required by the
school system each year. Day care, camp, and
sports physical forms may be faxed to save
you
